Program Performance Award: Service Connect Criminal Justice Realignment Program, County Health - Behavioral Health and Recovery Services

Service Connect of San Mateo County provides a range of services aimed at supporting former inmates as they re-enter the community. Service Connect is a collaboration between the San Mateo County Sheriff's Department, Correctional Health, Probation, Human Services Agency, and the San Mateo County Health. Our staff works across agency lines to reduce recidivism and connect clients with needed services.

Customer Service Award: WIC Program-Breastfeeding, County Health - Family Health Services

The program's mission is to serve low-income women throughout pregnancy and postpartum and their children up to age five by providing nutrition and breastfeeding education and resources to start families on the best track and greatly improve their life trajectory.

Green Award: GIS Division, Information Services Department

The GIS Division has been instrumental in deploying technology for the betterment of the community aligning perfectly to with ISD's mission to strengthen connections and support a healthy, safe, environmentally conscious, and collaborative community through: SMC HOME (Homeless Outreach and Mobile Engagement) app, COVID-19 and CZU fire responses, Water Quality Monitoring, the Property Map Portal, and the Open Data Portal, collectively demonstrating a commitment to leveraging technology for social good and environmental sustainability.

DEI Award: Oral Public Health Program, County Health - Family Health Services

The Oral Public Health Program (OPHP) works with community partners to improve the conditions for oral health in San Mateo County, particularly for those facing historical and systemic barriers to oral health.

The OPHP currently focuses on reaching pregnant people and children ages 0-5 and their parents/ caregivers who are on Health Plan of San Mateo (Medi-Cal) health insurance.
