Requesting San Mateo County CPS Records
Each county has its own Child Protective Services (CPS) agency and maintains its own records. If you are requesting records in a county other than San Mateo, please check with that specific county.
Instructions for requesting San Mateo County CPS records
- Complete the CPS Records Request Form in the link below. Fill it out with as much information as possible. Please be advised that incomplete or inaccurate information may delay the processing of your request.
- Once completed, the request form with supporting documents may be emailed (preferred method) as an attachment to: HSA_CFS_RedactRequests@smcgov.org

Procedures for receiving documents: emailed, in person or USPS certified mail.
- San Mateo County Children and Family Services staff will notify you via phone when your request is ready for mailing or pick-up.
- A fee of 10 cents per page is applied to all requests and must be received prior to mailing or at the time of pick-up. Foster youth under the age of 26 will not be charged per Foster Youth Rights. Additional USPS fees will be incurred if mailed.
- If requestor would like the record sent via USPS certified mail, the requestor must submit a notarized letter confirming identify with payment.
- Records will only be released with presentation of government issued identification to the requestor.

NOTE: Under Welfare and Institutions Code section 827, it is not legal to disseminate/distribute CPS records to any other party without a court order.
