2023 VAPAHCS Domiciliary Brochures

  • There is a NEW VIRTUAL Housing Resource group that providers and Veterans can call into. It has a QR code/dial in code and TEAMS code to make it the most accessible.
  • The San Jose Housing Resource Group has been added
  • HVEHF is no longer the provider for our Geriatric HCHV contract in San Jose. (we have not identified another provider at this time.)

For those whom may be new to VAPAHCS, the HCHV and GPD program are our homeless shelter and transitional shelter housing programs that we have agreements with.  Provider can call or Veterans can self refer. If you are having any difficulty getting a Veteran into one of the HCHV/GPD programs, please reach out to me to see if I can better assist you.
