Forms

In addition to the Application for Services, there are other forms which custodial parties and obligor parents may be asked to complete during the child support process. For convenience, these forms are available for printing from this website. These forms include:

CP Child Emancipation Letter (pdf 141 KB) (DCSS0124), which the custodial party will be required to complete when a child reaches the age of 18. This form is used to determine when the child will be legally emancipated and when it will be appropriate for the child support obligation to end.

Child Care Verification (pdf 295 KB) (Form DCSS0069), the person responsible for completing this form is the person who submits payments directly to the child care provider. A portion of the form must also be completed by the child care provider. This form is used by the SMC/DCSS in the calculation of a child support order based on the California Guideline.

Visitation Verification (pdf 318 KB) (Form DCSS0053), which is completed by the custodial party and/or the obligor parent and indicates the amount of obligor parent visitation during the previous 12 months. This form is used by the SMC/DCSS in the calculation of a child support order based on the California Guideline.

Case Closure Request (pdf 440kb) (Form DCSS0432), A custodial party may request their child support case be closed by providing our office with a signed letter. Please see the "close a case" section of this website for details on the criteria under which you may request case closure.

Income and Expense Declaration (FL-150), This is completed by either parent to inform the court of their current financial situation. This is also used by DCSS to establish/modify a child support amount. You may select the link to complete the form electronically.

Review and Adjustment (DCSS-0291), This is completed by either parent when there is a change in circumstances. You may select the link to either request a Review and Adjustment or to respond to a Review and adjustment electronically.
